Players please send the below information to:

  • player name
  • player age as of 12/31/2019
  • player birthdate
  • player current team and level (rec or select A, B, C)
  • what age they want to try out for (10u, 12u, 24u, 16u)
  • positions they play
  • other select sports/activities they play
  • Do they have teammates or friends also trying-out for Legends? What are their names?
  • Parent(s) contact info: name(s), address(es), e-mail(s), phone(s)
