I/we acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC, state, local and other public health authorities still recommend practicing social distancing.
 

I/we further acknowledge that Omaha Suburban Athletic Association (OSAA) has endeavored to put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
 

I/we further acknowledge that OSAA cannot guarantee that I/we will not become infected with the Coronavirus/ COVID-19. I/we understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to all those affiliated with OSAA, its directors, commissioners, coaches, assistants, volunteers, employees, players and their families.
 

I/we voluntarily seek services provided by OSAA and acknowledge that I/we am increasing my risk to exposure to the Coronavirus/COVID-19. I/we acknowledge that I/we must comply with all set procedures to reduce the spread while participating in OSAA activities.
 

I/we hereby release and agree to hold OSAA, its directors, commissioners, coaches, assistants, volunteers, employees, players and families harmless from, and waive on behalf of myself, my dependents, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the association, or that may otherwise arise in any way in connection with any services received from OSAA. I/we understand that this release discharges OSAA, its directors, commissioners, coaches, assistants, volunteers, employees, players and families from any liability or claim that I, my dependents, my heirs, or any personal representatives may have against OSAA, its directors, commissioners, coaches, assistants, volunteers, employees, players and families with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from OSAA. This liability waiver and release extends to OSAA together with all its directors, commissioners, coaches and assistants, volunteers, and employees.

By electing to participate in the 2020 Sports Season, I/we acknowledge that we have read, understood and agree to the provisions above.
